You're assuming that everything in the dictionary is
meant to be a "word". That's a false assumption. There are numerous examples in dictionaries that are not words, but simple interjections ("ooh", "ugh", "gosh", etc.)
In the specific case of "d'oh", the only reason it's even
in Webster's is because of The Simpsons, the expression having been popularized there.
