D2D often rejects epubs because their clients (especially Apple) have requirements that go far beyond epub validation. The main problem is that the title, subtitle, and author of the book on D2D must exactly match the wording on both the uploaded cover and on the book's title page. Early on in this business, I often omitted the subtitle on the cover, used a variation of the magazine cover in which the book first appeared as an article, or added a sales pitch such as was regularly used on the dustcover of hardcover novels. For Apple and apparently for many others, such variations are rejected as an epubcheck failure.

You might want to consider that possibility.
