Select it (the image) and set the semantics to cover.
Calibre puts a cover icon in the book browser.
The HTML file that calls that image, must also be set to cover. If there already is a Cover file, just edit the image tag to be the real image flagged above.
Another way is to simply copy and past the image into the MDE and Polish once the edit session is done. (then cleanup the leftovers in the book)
