If you used linked images and changed where they are stored you will see placeholders in Word, or any other program with images in it.
This is most unlikely to be caused by Calibre.

It always will if it's a linked image rather than an embedded copy and source path is changed or source renamed or removed.

Don't do that. That rarely is a solution.
Edit:
If you used relative paths for linked images in MS Word and change the document location you'll get placeholders as the path will be wrong.
Always embed (copy & paste or be careful with Insert From File options) images.
