Since it's impossible to create an epub file with an Adobe page-map.xml file that'll pass EPUBCheck, you could use my PageList plugin to add page numbers to the NCX file.
Unfortunately, AFAIK, there's not a single EPUB2 reader app that'll actually display NCX page numbers.

However, if you convert your epub2 book to an epub3 book with Kevin's ePub3-itizer plugin, it'll will convert the NCX pagelist to a NAV pagelist.
