The only time I've seen that happen with a Kindle was with a PW4 when a power glitch caused the computer to reboot while sending ebooks to the Kindle. A reconnect and a quick check with Nirsoft's USBDeview showed a connected unknown USB device with a device descriptor request failed message. After removing that device and reconnecting, I haven't seen an issue that swapping cables didn't fix.

I suppose I could get the same effect by removing the USB cable in the middle of a file transfer but prefer to safely eject and avoid potential issues.
