I've seen it several times and have reported it to Kobo when it happens. The problem is the usual one with an erratic problem not being easily reproducible making it a pain to fix. So far a restart has been the easiest solution since I have restart as a NickelMenu option. It has nothing to do with Adobe authorization since I've had it happen on a device that did not have activation.xml in the .adobe-digital-editions directory (device.xml will always be there unless you deliberately remove it).
Personally, I tend to find it rather humourous when an epub without DRM that has never been touched by an Adobe content server is reported as being protected by DRM. But as most of my friends and family would say, my sense of humour is definitely warped.
