The error in the OPs post indicates copy between opened file descriptors is failing. This means that after the files were successfuly opened, the kernel is failing to copy between the files. The only way that can happen is buggy file system drivers or some third party software interfering. Which is why I am enquiring about the location of the file being added and the calibre library.
