Add the custom columns to calibre's setup.
Add the needed columns used to KoboUtilities configuration.
Add the checkmark to Store current bookmarks on connect in Kobo Utilities. You may want to remove the Only if more recent checkmark.

I think there is a decent explanation for the needed setup in the Kobo Utilities plugin thread.
