What doesn't make sense to me is that it's only happening with books sent and converted with the extendtouch driver.

Have you tried Polishing the book in your library, having Polish update the cover and metadata with the version what shows in the details of your Library.
That will insert a actual 'COVER' (what you see is a simulated cover for books without
properly marked covers)
