I tried updating from saved metadata, but that isn't updated either without a chapter being added. It does change if I use the option to update metadata from the web site, but I don't want to have to do that.

Is it possible to get status changes to be recognized as legitimate updates on their own?

Update if New Chapters checks for updates by comparing the number of chapters. You can use Update ePub always, which will also update the title page.
