Vellum are doing it for a lot more than the images, for which there are solutions that don't need to know size of screen or @media
You don't unless the epub is a crazy format to start with.
Amazon creates functional a mobi/KF7 file if you have ancient. Otherwise you get a KF8 (azw3) or KFX file depending on download for USB transfer or whispernet.
We test buy for a Kindle that only does mobi and for a PW3 and KK3 if we do any new style, but we test locally first.

I removed most of it and the ebook is still fine.
