If I log in first, audiobooks.com wants me to reactivate my account for $15/month. Even if I put the Dreamscape book in my cart first, I can't enter the promo code with reactivating the account.
I suppose I could use a different email, but I'm too annoyed to bother.

Login to Audiobooks.com --> click on "My Account" in the upper right corner --> scroll to bottom of page and enter your promo code. The email instructions are for new customers.
