I think its a Kobo Aura 2nd. can't find a type indication on the device.
The model name and number should be on the end of the device with the USB port. But, there is a complication for the Aura Edition 2. There are two versions and they need different images. Unfortunately, the only way to tell is from the serial number. The first version has a "0" in the 7th character of the serial number and the second version has a "2". Or, if you know when it was bought, we can take a guess.
