As noted above, conversion from PDF to epub tends to be terrible.

When I have a need, I find modern versions of Microsoft Word do a a very decent job on many pdfs, particularly if the was originally generated by a computer (rather than a scan). This is particularly true for pdfs that were created from Word documents, of course, but in general, its pretty decent.

If Word produces a decent result, I save it as a docx and use Calibre's docx-to-epub conversion (which is very good). Sometimes it works, sometimes it doesn't but if it doesn't work, usually nothing else does either.
