He sent me the ePub version of his books that had the problem so I deleted the versions in MapleRead SE and loaded the new ones. All three books are fine. Thus the problem is apparently Amazon.

Now if only Amazon would let the buyer choose ePub or Mobi when they buy a book; better yet get rid of Mobi altogether and just use ePub for Kindle books.
Please load the ePub in the Calibre editor and using epubcheck, check for errors. I'm curious what the ePub is like.
