Well, he responded with this information:

"Amazon used to request I upload the .mobi file, but now they want .epub. I think they then still convert it to .mobi. I suspect you going back to .epub is one conversion too many somehow."

He sent me the ePub version of his books that had the problem so I deleted the versions in MapleRead SE and loaded the new ones. All three books are fine. Thus the problem is apparently Amazon.

Now if only Amazon would let the buyer choose ePub or Mobi when they buy a book; better yet get rid of Mobi altogether and just use ePub for Kindle books.
