A signout/signin will replace the database and should not require you to reload any sideloaded books though all reading status, annotations, etc. will be lost. Using Kobo Utilities to back up the database will help to reduce any future losses to a minimal level.

You can try using an SQLite utility to repair the database using vacuum and/or re-index won't hurt and might help but the chances are pretty slim.
