I think it's a combination of how Monterey works (it auto-opens anything in the downloads folder) and that my computer is set so book files are opened by Calibre. So if I download a DRM free book to the Downloads folder, the system sees that the file is an epub and opens Calibre automatically if it's closed.
The problem was that I had auto-add also set to the same folder, so when Calibre opened, it was receiving the file the computer was sending it and at the same time, recognizing a book that needed to be added, and adding it, so I'd end up with 2 copies of the same book.
You might want to get someone else running Monterey to double check and see if that's how it's working for them as well, but it's actually the first thing that makes sense on this.
One reason I think this is the answer is that it also auto-opens Adobe Digital Editions if I download a book with DRM. Another reason I'm pretty sure Monterey is to blame is that the book itself isn't disappearing from the Downloads folder even after it's adding to Calibre, now that I've moved auto-add to another folder.
