I'm going to try

First of all: Books that you get from calibre content server (using OPDS -> calibre local catalog from KOReader) cannot interact **at all** with the other methods.

Now the good part: wired and wireless transfers can be made compatible as long as you follow the instructions below:

The standalone way (IMHO the best way).

On calibre:

1. disable Kindle device plugin.
2. Use "connect to folder" to transfer documents via usb.

On KOReader:

1. setup the calibre plugin's inbox dir to the same folder where you transfer your books via cable.

The mixed solution:

On calibre:
1. change the device template to push books without "authors" subfolders.

On KOReader:

1. setup the calibre plugin's inbox dir to the same folder where kindle device plugin send books (that should be /mnt/us or /mnt/us/documents), I don't remember.

2. Confirm "I know what i'm doing" when prompted. The prompt is here because the required step in calibre above.

Do notice it is possible to use "the mixed solution" without changing device template but that will end with books stored without subfolders when sent wirelessly and with subfolders when transfered via usb. That's bad taste, not tested and could lead to undefined behaviour.
