You added a cover to CALIBRE, not the book(assumes you really used the MDE, not the book Editor).

You NOW need to do another step to EMBED the cover into the book.
Calibre has Polish (a toolbar Icon) configure to replace the cover.
There is also, the Modify EPUB plugin that can do that task.

OTOH If you really were using the Edit Book (T), the cover IMAGE and the file that contains it, need to be marked as cover (there is an icon next to those names IF that has been done. Right click on each:Mark as cover if needed)
