Again: both profiles and gestures do the same: execute a number of actions defined by the user.

The only difference: on gestures you assign that set of actions a nice gesture. On profiles you assign that set of actions a nice name.

If you find something that should be actionable and is not present yet as something you can add to your set of actions it is because it isn't a dispatchable action.

You're welcome to request a new action. See https://github.com/koreader/koreader/issues/7488 for instance.

But first make sure you're requesting something actionable (ie: you're able to trigger it from a menu).
