Did you REGENERATE the NCX with the tool? (issues with links are underlined in red when you open the tool. fix or regenerate)
the h# tag is just used as a marker for the tool. it does nothing to the TOC by itself.
My experience is Kobo's do not see a TOC (link) update
from the original import. You must delete the book on the Kobo (and lose your place).
