Another interesting bit. If I have the Calibre Editor re-do the ToC's exactly as before, save it, and turn off the Modify ePub option to remove bad ToC entries, when I send it back to my Kobo, it works flawlessly. With the version that Modify ePub stripped the ToC out of, it failed when it tried to get to where the toc.ncx would normally show up (just before the start of the text) -- "Oops! This document couldn't be opened). I'm assuming it's failure to read is because toc.ncx is required for EPUB2s.
