WHY all these why's? without specifying if you are using the main GUI or the web interface?

There IS a TOC editor tool from the main GUI (book list) if you add it to the toolbar.

There are NO standards for Annotations. My Kobo is not compatible with my Kindle and not with Calibre (where I want to fix things I noted while reading on my device).
