I've not tried a reader that supports TTS, but I was working on a dictionary app that used TTS.
I was just using the Google TTS engine.
There are settings under Settings > Accessibility > Text-to-speech output for default speech rate and pitch. An app can also override these.
Since my app is a language dictionary I have the speech rate set at 0.5
This would not be a good rate for reading a book!
