As far as I know, the calibre library files will never show the series/series index as part of the filename since that should be stored in the metadata for the library. When you save to disk or send to device, there are templates to control the output filenames.

I've attached an image of what I see in my calibre library and in the library folders when I converted epub to mobi. I've also attached an image for the save to disk template and the send to device template (the one in the device driver which overrides the default template when connected to a Kobo device).

When I did a save to disk using the following template: {author_sort:sublist(0,1,&)}/{title}/{author_sort:sublist(0,1,&)} - {series:| |}{series_index:0>5.2f| | - }{title} (use the first entry in author sort for the subdirectory, then the book title as a subdirectory and then the first entry in the author sort - series_seriesindex - title, I see the following books in those directories.

Sterling, J. - The Perfect Game 01.00 - La partita perfetta.mobi
Sterling, J. - The Perfect Game 02.00 - La svolta di gioco.mobi
Sterling, J. - The Perfect Game 03.00 - La partita piu dolce.mobi

Basically, either your save to disk or send to device template is garbage or your metadata is garbage.
