I often use iBooks to read epubs I create and I have never seen this behaviour. There are no fragments in the hrefs in the NCX nor the Nav on any of them and the TOC look and work properly.

So maybe there is some other cause for what you are seeing?

So I add ids to body tags and use that for toc. When splitting such files, the toc entry changes to the last split. Maybe that can be fixed sometime to retain the original toc entry.
