It’s almost certainly out of warranty, and is showing signs that it’s broken.
You could try fixing it, it doesn’t seem like the substrate is broken since the lines are uniform and straight. So maybe the screen just needs to be reconnected. Though you’d have to be comfortable opening the Forma and finding where the screen is plugged in and disconnecting ribbon cable. Even that isn’t guaranteed to fix the screen issue.
Not sure what could fix the button issue, it’s known to be difficult for some people but it sounds like yours is actually broken. Perhaps it’s just not connecting to whatever it hits when you press it, so opening and looking may present more information.
It’s also worth mentioning that the above may break everything even more. Though I’d question how usable it is right now. A sleep cover would solve waking it up, and nickelmenu can at least add a reboot and power off option. Though to turn it on would require you to plug it in. The black lines are not likely to be resolved with any amount of software tinkering though.
