4.31 finally busted my Kobo but good. After restarting it, the Kobo all of a sudden needed me to sign in and was acting like a brand new device.
I rolled back to firmware 4.30 and have reapplied patches.
Now, since the above happened, I have not added any books to my Kobo (all books on my Kobo are sideloaded. Every single one).
Over time, Kobo rebuilt my library. When I got signed out, the sideloaded books were not wiped from the device. Kobo just 'forgot' about them and slowly rebuilt its database.
Once that was all done, I noticed that in My Books, Series was blank. Collections showed all of my collection names. But they all show '0 books'
I hooked the Kobo back up to Calibre and clicked 'Update cached metadata on device.'
When I ejected, Series was repopulated.
Collections still shows all collection names, but each still lists '0 books' (see attached image)
I have tried restarting and have tried connecting to Calibre and clicking 'Update cached metadata on device' several times. Everything else is coming back together. But I can't seem to get collections rebuilt.
Do I have to delete all books and then sideload them again to get collections to work?

Yes, this is to be expected. The list of collections/shelves is
still in the database actually I think the list of shelves gets synced, but your re-imported books aren't assigned to them because Kobo had to rebuild that part of the database. Take a look and you'll see reading positions will have been reset too.
Calibre stores series data by default. But Calibre doesn't store any collection/shelf information without setting up the Kobo collection column. So if you don't have one, there is nothing for it to update.
