I'm going to keep monitoring it, but I tentatively think that just bypassing Calibre and dragging the file into the kindle folder worked. I didn't strip the metadata - I don't know how to do that, beyond manually erasing the visible bits via Calibre's Edit Metadata function. But fingers crossed, it wasn't necessary!
The book's been behaving itself for a bit now, so I'm hoping that whatever's wrong was fixed. If it starts misbehaving again, I'll try fixing the html.
