This one in particular was similar to standard format except folder was named OPS. Thus:

OPS/Images
OPS/Styles/
OPS/Text/
OPS/content.opf
OPS/toc.ncx

There is also originally "META-INF/com.apple.ibooks.display-options.xml"

Tested a few others w/o issue.
