Given the debug log it is almost certainly a corrupted calibre installation. There isn't any way I know of to determine exactly how it became corrupted.
One way it can happen is if the C: file system is corrupt, causing cruft to be written in various places. It is a good idea to check the C: file system. Right click on the C drive, pick Properties / Tools / Error checking.
