Be sure you wait for the Safe to remove (USB cable) message. USB transfers can be buffered by the OS and not truly completed when you click Eject. (I've see up to 30 seconds before Calibre even drops 'OnDevice' )
Also, Kobos have a' Importing' screen. I always wait until My Books screen shows
