It might mean that you are doing the "tilty-tilt" as you either put it to sleep or wake it. I use mine locked in portrait, and frequently it will be the wrong way up to what it was when I stopped reading. But, I had to close the cover, put it down wherever, pick it up and open the cover. There is enough movement there to confuse the sensor and software.
As to holding it with the buttons at the top, I'm know someone has said they use it like that. I don't, but, I frequently hold the ereader by the top-right corner. If it was in landscape mode, I would be able to reach a button to turn the page.
