Thanks everyone. I installed that version and it now works (I'm not sure what version I had as I thought I had installed the latest release).
I'm still unable to uninstall Nickelmenu following these directions:

I tried creating a text file simply called "uninstall with no content, but it just sits there and Nickelmenu does not uninstall.
I was also unable to trigger the failsafe.
