I'm a little surprised a moderator hasn't come around to warn that detailed help is not permitted on MobileRead and you should go to Harper's GitHub or Alf's blog, where you can post logs.
That said, some hopefully-not-to-detailed comments:

Not really. KFX ebooks will have the generic .azw extension when downloaded to K4Mac. Do you see a .voucher file in the _EBOK folder along with the .azw file? If there's a voucher file, you have KFX.

Do I need to go back to an older version of Kindle or Calibre?

Worth a try. Go back to Calibre 4.23 and Alf 6.8.1. If that works, you may be able to successfully upgrade back to current versions.

Everything was working properly before the 1.32 update appeared.

Try doing a clean install of 1.31. Delete the app and User > Library > Application Support >
Kindle folder and/or Application Support > Containers >
com.amazon.Kindle.

I'm not sure you'd necessarily get an error message if chmod failed, though the fact the program crashes on exit is a good sign. See above note about the .voucher file. If you see a .voucher file, you're getting KFX.

... Then added book to calibre and it added as an AWZ and shows the title and cover properly, so it appears KFX is indeed disabled.

Actually, no. KFX will still have .azw extension in K4Mac and it will show title and cover in Calibre even though it's still locked by DRM.
If going back to Calibre 4.23 and Alf 6.8.1 doesn't work, and if a clean install of K4Mac doesn't work, please try Harper's GitHub or Alf's blog.
