That suggests that the epub you're updating was changed by something else and FFF can't find the existing chapters. Which would be unrelated to Browser Cache.
We've seen that before with users who choose to 'clean up' their ebooks a lot. The only specific examples I remember right this second are Sigil used to change the internal dir structure an break anthologies and converting to non-epub and back changes all the internal file names.
