UPDATE: I worked out that the problem was in my keyboard - my cat had pressed the "Record Macro" function and then, several hours later, I had searched for "generate covers plugin" and closed a heap of tabs. This was all recorded, including timings, which is why it took hours to happen. I'm still not sure what actually triggers the macro to run, but I was able to factory-erase the keyboard and the problem no longer occurs.

Sorry to have thought that the plugin was at fault - I guess the warning in calibre about untrusted plugins got stuck in my head.
