Images first: I don't think that the pages made into images, placed in html files are going to disturb readers in this case as they are only the front-matter pages (copyright, dedication, title page) and disturb more broken.
As for the css. I hesitate to post the problematic html portion of the book as it is fairly long. Would a short section of same do?
Similarly what would you need from the css. It too is relatively long and I suspect mostly irrelevant to this issue.
