I don't have to imagine. We lived it with our landlines decades ago. "Local" calls (included in the monthly service charge) were limited to a small radius from the registered line's location. Calls outside of that radius were subject to surcharges based on category of "toll" (intrastate with limits), "long-distance" (interstate), or "international." These surcharges were in addition to the flat monthly service and calculated on a per-minute basis. And there were numerous phone companies throughout the U.S., even between counties in the same area of a state.
I remember well "toll" charges for simply calling from one location to another within the same city.
