Use docx input to convert to epub2 in Calibre.
No page styles, use paragraph styles, no headers or footers. Only inline italic, bold, superscript and subscript formatting. Everything else a paragraph style. No inherited styles.
Make any headings not in TOC / Contents have no heading level, level = body text.
Contents page no page numbers and delete automatic TOC, make a bookmark at each heading and a link to it.
All heading styles in TOC/Contents should insert Page Break before.
Save As an extra docx. Use the current native format in Word or Writer to Edit.
If I'm wanting a PDF I use the WP source and change page size, add custom format pages at start and end, add page numbers and change format of Contents, add headers and footers, use different resolution images and may change paragraph styles and use inherited styles.
