Technically, it means Sigil
won't make/save an epub with iTunesArtwork file. Just like it won't make/retain any other unmanifested files in an epub.
As I mentioned before, your best solution here is to use a third-party output plugin to add the iTunesArtwork file to your epubs whenever you're done editing them in Sigil.
