I converted a word document to AZW3 file but the cover page generated by Calibre doesn't appear in the Table of Contents. The page is there at the start of the book (it also shows in the thumbnail) but there seems to be no way to add it to the TOC. (I also checked the option of 'manually fine-tune the TOC after the conversion is completed' but it didn't show the option of adding the cover page.)
