The next thought was whether you were using the webcache. The obvious thing would be that FFF would not find it in the cache, you would look at the page and say "But it is there". That updates the cache and hence FFF finds the update when you try again.

Also, there is the issue that the FFN site metadata often doesn't update to reflect a new chapter count for an hour or more even when the new chapter is visible on the site.
Until the metadata updates on the FFN website to reflect the increased chapter count, FFF won't find it even if the page is in the browser cache (i.e. you will get an error that the fic doesn't need to be updated because it already has XX chapters).
I usually give it at least an hour after the new chapter shows up on the FFN website before I try to run an update in FFF. I have a couple of fics (pretty long ones) I follow that I know from experience may take up to 3-4 hours for the metadata to update.
