Weird occurrence when changing epub cover through Calibre Editor. Looking for help!

As per my title, I've been trying to change covers to a number of epub files through the built-in Calibre editor and a very very weird malfunctioning keeps happening. I will now break down the whole process to explain the issue as clearly as I can:
Set-up: the external metadata for my ebooks have no cover, my epub files include no internal images, an HTML cover page already exists in each file but of course it links to no image, the embedded metadata are set to be updated from the external ones whenever the epub file is opened for modification.
Editing process A: I open the main Calibre programme, edit the external metadata, and add a cover image from my local files to the ebook. However, whenever I re-open the epub file through the ebook editor, the new cover will be saved into a dedicated OEBPS/metadata/metadata location rather than into the standard OEBPS/images folder, and the new cover file won't be automatically linked into the HTML cover page.
Editing process B: going backwards, I open the epub file through the dedicated editor, add a cover image through the dedicated command, save & exit, use the main Calibre programme to edit the external metadata, and use the dedicated command to import the cover image from the epub file itself to the external metadata. However, whenever I re-open the epub file through the ebook editor, it turns out that the cover image has been duplicated; one copy is properly linked into the HTML cover page, the other one just lays around unlinked.
Is anyone capable to deduce what am I doing wrong and offer some advice? I would be really really grateful.
