There are three kindle of fixed-layout Kindle books: print replica, comics, and children's books. Print replica is based on PDF. The other two kinds can be manually sourced as an EPUB by following the Kindle Publishing Guidelines or they can be produced by various tools: Kindle Comic Creator, Kindle Kids Book Creator, and Kindle Create.
If you create the fixed-layout book using an EPUB then it is recommended by Amazon that you publish using that. You can create a Master MOBI from the EPUB using the Kindle Previewer and use that for testing, but Amazon no longer prefers you to publish using that, although they currently still accept it as far as I know.
Do not try to publish a MOBI file produced using calibre. It it fine for use on a Kindle but lacks internal structures needed for a properly published Kindle book.
