I looked at the book and sent you a modified copy. Best I could do in the 15 minutes I allowed. Basically, the toc.ncx had issues with ids that did not exist in the epub and duplicated playorders. There were good chunks of epub3 only code in what showed as a epub2 file. The first pass through it, epubcheck pretty much upchucked. After correcting most of those errors (the inventors of regex be praised!), it seems much happier now.
