I uninstalled the extended plugin as well as the KePub output plugin and converted the book to epub and it installed just fine (though I didn't like the formatting--it didn't have a TOC. It installed just fine on my Kindle, too, and the TOC did show there. So, I reinstalled the KePub output plugin, converted the epub to KePub and it installed without any problem (and the TOC is there.)
