I am still looking into the original problem, but one thing I noticed wrong with those files is that "<" and ">" characters in definitions are not being escaped, leading to text being treated as HTML tags. For example:

welcome / make <a person> welcome / receive <a person> with delight.


Added: Actually I did find the problem and it is related to the above issue. In the entry that is failing the closing tag (</idx:entry>) is being lost because the definition is truncated in the middle of text with unescaped characters:
